A full-term initial evaluation at Miracle Leaf® is up to $199, and each recertification is up to $149. Those are the maximum prices allowed across the network. Shorter certification options start at $99. Every Miracle Leaf clinic is independently operated and each franchisee sets its own price at or below the network maximum, so pricing varies by clinic. State program fees are separate from the visit price. Those fees are set and collected by the state, not by Miracle Leaf, and they are paid directly to the state program.
The full-term evaluation covers the entire 210-day certification period, which is the longest a Florida physician may issue under Fla. Stat. §381.986. It is not a shorter certification that sends you back for another paid visit in a few weeks.
That distinction is the one most price comparisons miss. A lower advertised visit price can cover a shorter period, so the cheaper first visit becomes the more expensive year. What matters is the cost per certification period, not the number on the first invoice.
Shorter options are available and start at $99. They cover about three months instead of the full 210 days and do not include a Request for Exemption (RFE). For most patients an RFE is not part of a standard certification, so the shorter option is a real choice rather than a stripped one. Ask your clinic which fits you.
A medical marijuana card cost has two separate parts. First is the clinic visit or evaluation fee, up to $199 for a full-term initial evaluation and up to $149 for a recertification, with shorter options from $99. Second is the state program fee paid directly to the state, which is $77.75 a year in Florida, $30 for a five-year card in Georgia, and none in Texas.

Evaluation figures are the network maximum. Each independently operated clinic sets its own price at or below that maximum, so your total may be lower. Florida also charges a separate $15 state fee to replace a lost card.
The cost covers two things. The clinic visit includes your physician evaluation and time to ask the doctor questions about your certification at no extra charge. The separate state program fee covers your state registration or registry card. Any product you buy later at a licensed dispensary is a separate cost. There is no charge from Miracle Leaf® for Florida exemptions.

Georgia's medical cannabis registry charges $30 for a five-year card under O.C.G.A. 31-2A-18. Texas TCUP has no state card or fee under HSC Chapter 487.
Advertised visit prices are not the same thing as what you will pay. Compare the full first-year total instead, and check four things for any provider you consider.
Cost is not only what you pay per visit. It is also what happens when your physician moves on. Because Miracle Leaf clinics share one network, your records stay with the network rather than with a single office, and another network physician can pick up your care with your full history already in hand instead of restarting you as a new patient. Each physician still makes an independent clinical judgment when certifying you, as Florida law requires.
State programs differ, and the visit price can vary by store.
