About Miracle Leaf® Bonita Springs
If you live out here, you know Bonita Beach Rd SE: the road that runs east from the Gulf beaches toward the Interstate 75 interchange. We set up our Miracle Leaf® office right on that artery in the southwest corner of Lee County so you do not have to chase down a certification appointment across the county. Neighbors come to us from Estero, San Carlos Park, Bonita Bay, and the gated communities along the coast between Naples and Fort Myers. We do one thing here: the physician evaluation for the Florida Medical Marijuana Use Registry. We are a doctor's office, not a dispensary, and no cannabis is stocked or sold at this address.
The Bonita Springs neighbors we see
This stretch of southwest Florida is spread out, and we like that we sit at a crossing most of you already pass through. Beach-side folks off Hickory Boulevard, families inland near Imperial Parkway, and the year-round and seasonal households across Estero all funnel onto the same Bonita Beach Rd and US-41. Bonita Springs runs older than most of the state, and a good share of the people we see are retirees and the area's many military veterans managing chronic pain or PTSD. We kept the office close to the I-75 ramps so the drive stays short whether you are coming down from Estero or up from the north Naples line.
How your certification visit works
When you come in, one of our physicians sits down with you, goes over your medical history, and decides whether your condition is recognized under Florida law. If it is, the doctor records a certification in the state registry, and that record is what lets you register with the state yourself. We handle both first-time certifications and renewals, so whether this is your first visit or your yearly one, you are in the right place.
What to bring
Bring a valid Florida photo ID, or other proof of a Florida address, along with any records that document your condition. We see plenty of snowbirds out here, so if you only live in Bonita Springs part of the year, carry proof of your current Florida address too. The registry checks residency before it clears an application, and we would rather you bring it once than make a second trip.
A clinic, not a dispensary
Florida keeps the medical step and the retail step apart, and we want to be clear about which one we are. Under Fla. Stat. 381.986 a physician must certify you first. Only afterward, once the state approves your registration, can you buy from a licensed Medical Marijuana Treatment Center, which is a separate business. There are dispensaries right on Bonita Beach Rd if you qualify, but we handle only the certification side, and nothing more.

What to bring
- Government-issued photo ID showing your state residency (driver’s license, passport, or state ID).
- Medical records documenting your qualifying condition. Recent diagnosis notes, treatment records, imaging reports, or prescriptions all help the physician confirm eligibility.
- List of current medications including supplements and over-the-counter treatments.
Before you arrive
- Plan to arrive 10 to 15 minutes early to complete intake paperwork.
- Wear comfortable clothing. The visit involves a routine medical conversation, not a physical procedure.
- Bring payment for the evaluation fee. The clinic accepts most major payment methods. State program fees are paid separately to the state.
During the visit
A licensed physician reviews your records, discusses your symptoms, and determines whether you meet a qualifying condition. A typical visit takes 30 to 45 minutes. If you qualify, the physician completes the certification step required by your state program.
After the visit
Approved patients receive instructions for state registration. You purchase products separately from a state-licensed treatment center. Miracle Leaf® clinics do not dispense or sell cannabis products.

Initial evaluation starting at $199. Card renewal $149. Pricing varies by state and location, so call the clinic for full details.
The service is all-inclusive. The evaluation fee covers the full visit, and the only separate cost is the state of Florida registry fee, which patients pay directly to the state.
In Florida there is no extra charge for exemptions. Patients can speak with the doctor about their needs at any time at no additional cost.
