About Miracle Leaf® Davie
You'll find our Miracle Leaf® office at 2269 University Dr in Davie, right on the western Broward stretch of University Drive, that college-town belt running past Nova Southeastern University and the student and faculty neighborhoods around it. I-595 and the Florida Turnpike both feed the area, so if you're coming from Plantation, Cooper City, or western Sunrise you can drop in without a long detour. We certify patients for Florida's program here. We don't sell any cannabis.
A limited schedule, so give us a call first
We'll be honest with you: this office doesn't keep full daily hours. We're closed Wednesday and Thursday, and we close early on Saturday, so the single most useful thing you can do is call ahead and lock in a time before you drive over. Once you're in, one of our physicians checks whether your record meets a qualifying condition under Florida law and, where it does, records the certification that makes your state registration possible.
What to bring with you
Bring a Florida ID or other proof of residency and any medical records that describe your condition. You can fill out the intake form from home through our portal to keep your in-office time short. There's parking right on-site.
The clinic versus the treatment center
Florida law keeps two roles separate: the physician who certifies you, and the licensed Medical Marijuana Treatment Center that later sells product once the state clears your registration. We fill only the first role.
State physician registry verification
The Florida Department of Health Office of Medical Marijuana Use maintains a public registry of qualified physicians. Before your visit, look up your certifying physician through the OMMU qualified physician search. Access date: 2026-06-01.

What to bring
- Government-issued photo ID showing your state residency (driver’s license, passport, or state ID).
- Medical records documenting your qualifying condition. Recent diagnosis notes, treatment records, imaging reports, or prescriptions all help the physician confirm eligibility.
- List of current medications including supplements and over-the-counter treatments.
Before you arrive
- Plan to arrive 10 to 15 minutes early to complete intake paperwork.
- Wear comfortable clothing. The visit involves a routine medical conversation, not a physical procedure.
- Bring payment for the evaluation fee. The clinic accepts most major payment methods. State program fees are paid separately to the state.
During the visit
A licensed physician reviews your records, discusses your symptoms, and determines whether you meet a qualifying condition. A typical visit takes 30 to 45 minutes. If you qualify, the physician completes the certification step required by your state program.
After the visit
Approved patients receive instructions for state registration. You purchase products separately from a state-licensed treatment center. Miracle Leaf® clinics do not dispense or sell cannabis products.

Initial evaluation starting at $199. Card renewal $149. Pricing varies by state and location, so call the clinic for full details.
The service is all-inclusive. The evaluation fee covers the full visit, and the only separate cost is the state of Florida registry fee, which patients pay directly to the state.
In Florida there is no extra charge for exemptions. Patients can speak with the doctor about their needs at any time at no additional cost.
