About Miracle Leaf® Jacksonville

San Marco is one of Jacksonville's oldest planned neighborhoods, laid out in the 1920s on the south bank of the St. Johns River around the landmark lion-fountain square and its tree-lined streets. We put our Miracle Leaf® office on Hendricks Ave, the district's main north-south street, so we sit within easy reach of San Marco Square, the San Jose corridor, and the broader Southbank. Neighbors come to us for the physician evaluation for the Florida Medical Marijuana Use Registry. We do not sell any cannabis here.

Rooted in the Southbank neighborhoods

Hendricks Ave runs from the riverfront down through San Marco into the San Jose neighborhoods, paralleling Philips Hwy and feeding the I-95 ramps that connect the south bank to the rest of Duval County. That keeps us handy for the historic blocks of San Marco, the older San Jose estates, and the southside neighborhoods along the river's south shore. Jacksonville is a deep military town, with Naval Air Station Jacksonville and Naval Station Mayport nearby and one of the largest veteran populations in the state, so we see a lot of veterans and we are glad to serve them close to home.

What your visit involves

When you come in, one of our physicians reviews your medical history and decides whether your condition is recognized under Florida law. If it qualifies, that gets recorded as a certification in the state registry, which is what clears you to complete your own state registration. We serve both new patients and folks coming back for their yearly renewal.

What to bring

Bring a Florida photo ID or other proof of a Florida address, plus records that document your condition. If you live in the Jacksonville area only part of the year, bring current proof of a Florida address too, since the registry will not approve an application without confirmed residency. Give us a call first if you are not sure what counts and we will tell you exactly what to bring.

A clinic, not a dispensary

Under Fla. Stat. 381.986, certification by a physician must come before any purchase, and certification is the only step we handle. You buy the product afterward from a state-licensed Medical Marijuana Treatment Center, a separate licensed business, once the state approves your registration. There is a dispensary right beside us on Hendricks if you qualify, but the law keeps it separate from the office that certifies you.

What to bring

  • Government-issued photo ID showing your state residency (driver’s license, passport, or state ID).
  • Medical records documenting your qualifying condition. Recent diagnosis notes, treatment records, imaging reports, or prescriptions all help the physician confirm eligibility.
  • List of current medications including supplements and over-the-counter treatments.

Before you arrive

  • Plan to arrive 10 to 15 minutes early to complete intake paperwork.
  • Wear comfortable clothing. The visit involves a routine medical conversation, not a physical procedure.
  • Bring payment for the evaluation fee. The clinic accepts most major payment methods. State program fees are paid separately to the state.

During the visit

A licensed physician reviews your records, discusses your symptoms, and determines whether you meet a qualifying condition. The physician evaluation itself takes about 15 minutes. If you qualify, the physician completes the certification step required by your state program.

After the visit

Miracle Leaf® specializes in guiding patients through each step of the process during and after the appointment. Miracle Leaf® has over 8 years of experience working with the Florida Medical Marijuana Use Registry (MMUR).

Approved patients receive instructions for state registration. You purchase products separately from a state-licensed treatment center. Miracle Leaf® clinics do not dispense or sell cannabis products.

No. The Hendricks Ave office is a physician certification clinic. A qualified doctor evaluates whether you meet a qualifying condition under Florida law and, if appropriate, records a certification in the state registry. Any cannabis product is purchased separately at a licensed Medical Marijuana Treatment Center.

Does Florida accept an out-of-state medical card?
No. Florida does not recognize medical cannabis cards issued by other states. Part-year and seasonal residents must complete a Florida certification and register with the state under the same rules as permanent residents.

The service is all-inclusive. The evaluation fee covers the full visit, and the only separate cost is the state of Florida registry fee, which patients pay directly to the state.

In Florida there is no extra charge for exemptions. Patients can speak with the doctor about their needs at any time at no additional cost.
