About this clinic
About Miracle Leaf® Key West
If you know Key West, you know Duval St, the strip that cuts across Old Town from the Gulf side to the Atlantic side, right where US-1 finally runs out at the southernmost point of the continental United States. Our Miracle Leaf® office sits at 126 Duval St, on that central pedestrian stretch. The island's geography is the whole story here: cut off from the mainland by more than a hundred miles of bridges, you'd otherwise face a long drive up the Keys for an evaluation. We keep the Florida Medical Marijuana Use Registry physician evaluation right here on the island. We don't sell any cannabis.
An island clinic for the lower Keys
Duval St is the one address nearly everyone in Key West can place, running the short width of the island between the two waterfronts. From Old Town you can reach us on foot or with a quick drive from any neighborhood, and we give folks down in the lower Keys a certification option that doesn't mean crossing the Seven Mile Bridge toward the mainland.
The certification step
One of our physicians reviews your medical history and decides whether your condition is recognized under Florida law. A qualifying finding gets recorded as a certification in the state registry, which is what makes you eligible to register with the state. We handle both new certifications and renewals.
What to bring
Bring a Florida photo ID or other proof of a Florida address, along with records that document your condition. Key West has a large transient and seasonal crowd, so if you live on the island only part of the year, carry current Florida-address proof too. The registry requires verified residency before it approves an application.
Why we're a clinic and not a shop
Florida law at Fla. Stat. 381.986 separates the medical certification from the retail purchase. We perform the certification. The purchase happens afterward at a state-licensed Medical Marijuana Treatment Center, a separate business from ours, once the state approves your registration.

The Florida Department of Health Office of Medical Marijuana Use maintains a public registry of qualified physicians. Before your visit, look up your certifying physician through the OMMU qualified physician search. Access date: 2026-06-01.

Preparing for Your Appointment
What to bring
- Government-issued photo ID showing your state residency (driver’s license, passport, or state ID).
- Medical records documenting your qualifying condition. Recent diagnosis notes, treatment records, imaging reports, or prescriptions all help the physician confirm eligibility.
- List of current medications including supplements and over-the-counter treatments.
Before you arrive
- Plan to arrive 10 to 15 minutes early to complete intake paperwork.
- Wear comfortable clothing. The visit involves a routine medical conversation, not a physical procedure.
- Bring payment for the evaluation fee. The clinic accepts most major payment methods. State program fees are paid separately to the state.
During the visit
A licensed physician reviews your records, discusses your symptoms, and determines whether you meet a qualifying condition. The physician evaluation itself takes about 15 minutes. If you qualify, the physician completes the certification step required by your state program.
After the visit
Miracle Leaf® specializes in guiding patients through each step of the process during and after the appointment. Miracle Leaf® has over 8 years of experience working with the Florida Medical Marijuana Use Registry (MMUR).
Approved patients receive instructions for state registration. You purchase products separately from a state-licensed treatment center. Miracle Leaf® clinics do not dispense or sell cannabis products.

Pricing
Initial evaluation starting at $199. Card renewal $149. Pricing varies by state and location, so call the clinic for full details.
The service is all-inclusive. The evaluation fee covers the full visit, and the only separate cost is the state of Florida registry fee, which patients pay directly to the state.
In Florida there is no extra charge for exemptions. Patients can speak with the doctor about their needs at any time at no additional cost.
