About this clinic

About Miracle Leaf® Pembroke Pines

Our Miracle Leaf® office runs along Pines Boulevard at 12510 Pines Blvd, in the master-planned heart of southwest Broward near the Pembroke Lakes Mall and the Town Center shops everyone knows. I-75 is a short drive west, so we're an easy reach not just for Pembroke Pines neighbors but for folks in Miramar, Cooper City, and Weston coming along the corridor. You come to us for your physician certification. We don't sell any cannabis here.

Evening hours for a town that works elsewhere

We know Pembroke Pines is a community where most folks work somewhere else, so we keep our weekday hours running into the evening and stay open a full Saturday too. When you come in, one of our physicians reviews your medical history, decides whether a qualifying condition under Florida law is met, and enters the certification that lets you register with the state. We see first-time patients and renewals both.

What to bring

Bring a Florida ID or other proof of residency and the records that document your condition. If you can, finish the portal intake before you head over so your visit goes quicker. There's surface parking right here at the Pines Boulevard office.

A clinic, not a dispensary

Florida keeps the physician's certification and the retail purchase as two separate licensed steps under Fla. Stat. 381.986. We handle the certification. Pembroke Pines doesn't allow dispensaries inside city limits, so once the state approves your registration you'll buy your product at a licensed Medical Marijuana Treatment Center in a neighboring city, a separate business from ours.

State physician registry verification

The Florida Department of Health Office of Medical Marijuana Use maintains a public registry of qualified physicians. Before your visit, look up your certifying physician through the OMMU qualified physician search. Access date: 2026-06-01.

Review note

Plan your visit

Preparing for Your Appointment

What to bring

  • Government-issued photo ID showing your state residency (driver’s license, passport, or state ID).
  • Medical records documenting your qualifying condition. Recent diagnosis notes, treatment records, imaging reports, or prescriptions all help the physician confirm eligibility.
  • List of current medications including supplements and over-the-counter treatments.

Before you arrive

  • Plan to arrive 10 to 15 minutes early to complete intake paperwork.
  • Wear comfortable clothing. The visit involves a routine medical conversation, not a physical procedure.
  • Bring payment for the evaluation fee. The clinic accepts most major payment methods. State program fees are paid separately to the state.

During the visit

A licensed physician reviews your records, discusses your symptoms, and determines whether you meet a qualifying condition. The physician evaluation itself takes about 15 minutes. If you qualify, the physician completes the certification step required by your state program.

After the visit

Miracle Leaf® specializes in guiding patients through each step of the process during and after the appointment. Miracle Leaf® has over 8 years of experience working with the Florida Medical Marijuana Use Registry (MMUR).

Approved patients receive instructions for state registration. You purchase products separately from a state-licensed treatment center. Miracle Leaf® clinics do not dispense or sell cannabis products.
